YOU CAN NOW ONLY OPT IN TO ONE OR MORE OF THE COLLECTIVE PROCEEDINGS IF BOTH OF THE FOLLOWING APPLY:

1. You were not living in the UK on 1 July 2026,

and

2. You meet at least one of these conditions:

You entered into a Combined Handset and Airtime Contract before 1 October 2015 and, after the expiry of the Minimum Term of that contract, made one or more periodic payments in excess of the SIM Only Price on or after 1 October 2015; and/or

Between 31 March 2025 and 1July 2026, you made one or more periodic payments in excess of the SIM Only Price after the expiry of the Minimum Term of a Combined Handset and Airtime Contract. It does not matter whether you also made such payments on or before 31 March 2025.

If you meet either one of the criteria above, we refer to you as part of the “Amendment Group”

I confirm that, in relation to each proceeding selected above, I am a member of the Amendment Group and satisfy the relevant Class Definition because:

I entered into at least one Combined Handset and Airtime Contract with the relevant Defendant (and/or any other company which was, from time to time, within the same corporate group as that Defendant) under an Included Brand before 1 October 2015 and, after the expiry of the Minimum Term of that contract, made one or more periodic payments in excess of the SIM Only Price on or after 1 October 2015; and/or
Between 31 March 2025 and 1 July 2026, I made one or more periodic payments in excess of the SIM Only Price after the expiry of the Minimum Term of at least one Combined Handset and Airtime Contract with the relevant Defendant (and/or any other company which was, from time to time, within the same corporate group as that Defendant) under an Included Brand.

“Included Brand” means: In the claim against Vodafone: the “Vodafone” brand;

In the claim against EE: the “EE”, “Orange” or “T-Mobile” brands;
In the claim against Three: the “Three” brand; and
In the claim against O2: the “O2” brand.

“Minimum Term” means the period during which the customer is liable to pay a charge for early termination of a Combined Handset and Airtime Contract.

“SIM Only Price” means the periodic charge payable in respect of Airtime Services only, without the supply of a handset.

I acknowledge that, if this form is submitted after 21 October 2026, I will not be able to opt in to the relevant class or classes without the permission of the Competition Appeal Tribunal and there is no guarantee this permission will be given.
